Absences / Medical & Parent Notes

·       Students should see teacher upon returning from any absence to check if any assignments need to be made up.

·       Students will be required to make up any graded assignment missed while absent or on a medical / parent note.  All assignments (except written tests) can be made up or retaken on Thursdays after school.

·       A parent note may excuse students from participating in PE for a maximum of three days; however, students are still expected to dress out unless there are special circumstances.

·       If the illness or injury persists longer than three days, a medical note is required.

·       Medical / Parent notes should be given to students PE teacher immediately upon arriving to class.

·       Parent notes must be written by a legal guardian.

·       Medical / Parent notes must include the following information:

o  Date written

o  Dates of excuse

o  Specific limitations

o  Parent daytime phone number (if parent note)

·       Medical notes must be written by a medical doctor on their office form / letterhead.

·       If the medical excuse is less than six weeks:

o  Students who are unable to dress will report to roll call numbers at the beginning of the period

o  Teacher will decide whether they stay for class instruction or report to the library

§  Students who are sent to the library will complete a written assignment counted towards their citizenship grade

·       If the medical excuse is more than six weeks the student will be taken out of PE and placed in an elective class/TA

·       It is the students responsibility to make sure all missed assignments are made up
