Parent/Medical Notes: 

If your son or daughter is sick or injured, a parent note can excuse a students for up to three days.  Riverside Unified School District policy requires a doctor's note to excuse a student after the third day.

Please make sure you include the date, specific limitations, and a daytime phone number where you can be contacted on the note.
